There is no such rule. In particular, a card with the same name can appear more than once in the command zone, as is the case, for example, with two cards named Lost Mine of Phandelver (at least if they're owned by different players) (C.R. 309.2a), or by two Ferris Wheel cards owned by different players (C.R. 718.6), or two vanguard cards with the same name (C.R. 312.2).
With Rakdos Charm's third mode, creatures each deal 1 damage to their controller simultaneously.
Thus, Ob Nixilis's triggered ability will trigger only if exactly 1 life is lost by any "opponen[t]" this way (which can generally happen only if any "opponen[t]" controls exactly one creature), and then only once even if multiple "opponents" lose life this way (C.R. 608.2c, 608.2f, 603.2c; see also C.R. 700.1).
For purposes of Errant and Giada, just because an effect may allow a spell without flash to be cast "as though [it] had flash" doesn't mean that spell has flash. The spell is treated as having flash only for purposes of the effect, and not for other purposes, including for purposes of Errant and Giada (C.R. 609.4).
In general, an effect that creates "a token that's a copy of" something, with or without exceptions (C.R. 707.9), defines that token's initial values (C.R. 111.3). Such values "are functionally equivalent to the characteristic values that are printed on a card" (C.R. 111.3). They are also "the values of the characteristics defined by" that effect — the values of the "actual object" — for purposes of C.R. 613.1.
Other effects, such as those of Opalescence or Starfield of Nyx, can modify these initial values (C.R. 613.1; see, e.g., C.R. 613.1d, 613.1g).
Among other reasons, a "permanent is removed from combat ... if it’s a planeswalker that’s being attacked and stops being a planeswalker" (C.R. 506.4). "A planeswalker ... that's removed from combat stops being attacked" (C.R. 506.4). A creature that was attacking a planeswalker that's removed from combat isn't removed from combat too (C.R. 506.4c).
A permanent that isn't a planeswalker doesn't lose any loyalty counters for being dealt damage (C.R. 120.3, especially C.R. 120.3c), but it can still lose loyalty counters by other means, including by one of its "-N" loyalty abilities, if any, being activated (C.R. 606.4, 107.7) or by spells such as Aether Snap or abilities such as Vampire Hexmage's activated ability. However, state-based actions send only planeswalkers, not other permanents, to a graveyard for having no loyalty counters on them (C.R. 704.5i, 306.5c; see also C.R. 306.5).
Prowess is a triggered ability that gives a creature +1/+1 (C.R. 702.108a). Proliferate is a keyword action that adds counters (C.R. 701.27a).
However, an effect that says "get[s] +N/+M" or "get[s] -N/-M" (as with prowess) is not the same as putting a counter on anything (C.R. 613.4c distinguishes between "effects" and "counters" of the kind relevant here).
In game terms, putting counters on an object includes—
- "putting counters on that object while it's on the battlefield", regardless of what verb is used to refer to that action (whether "put", "move" [C.R. 122.5], "double" [C.R. 701.9e], "give", or some other verb) (see also C.R. 701.1), and
- the case of "an object that's given counters as it enters the battlefield" (e.g., "enters the battlefield with ... counters"; see also C.R. 122.6a)
The effect of Insurrection can expire even while the permanent it affects is phased out (C.R. 702.26f). Even so, the permanent will phase in during the next untap step of whoever controlled it when it phased out (C.R. 502.1).
Warp World says, "Each player shuffles all permanents they own into their library, then reveals THAT MANY cards from the top of their library." The counting of the tokens doesn't appear to treat them as cards. A certain number of tokens go into the library, and then THAT MANY cards-- actual cards-- are put onto the battlefield. Right???? Or am I missing something?

Im one of that player's opponent here (EDIT: Unless that person used this thread as an exemple? I was refered to this thread by my opponent though)... One of the things my opponent isnt saying here is that Goblin Trashmaster was the 3/3 creature and I blocked it with an indestructible creature. My opponent also had Ib Halfheart, Goblin Tactician on the field, which says:
"Whenever another Goblin you control becomes blocked, sacrifice it. If you do, it deals 4 damage to each creature blocking it."
So technically, the attacking Goblin Trashmaster got sacrificed automatically. The question should be: Does the +1/+1 boost from Trashmaster stays on my opponent other creatures if Trashmaster got sacrificed before damage calcultation or if it was sacrificed during the damage calculation stage of the fight?
As soon as Goblin Trashmaster leaves the battlefield, it will no longer give +1/+1 to any creatures anymore, since it's no longer on the battlefield (C.R. 604.1, 613.1b, 113.6).

If a triggered ability (such as Ib Halfheart's first ability) triggers during a step or phase (e.g., during the declare blockers step), it will get to resolve during that same step or phase (C.R. 603.3, 117.4).
In particular, in the case of Ib Halfheart's first ability, the creature it refers to will be sacrificed when the ability resolves, during the declare blockers step and before combat damage is assigned (C.R. 506.1).
In general, an object's counters are not retained as the object moves from one zone to another, including from exile to a hand, a library, or the stack (C.R. 122.2). The counters are not "removed" this way (C.R. 122.2). (An exception is found in Skullbriar, the Walking Grave [C.R. 101.1].)
Despite Solemnity, if you exile a card due to suspend you still put the appropriate number of time counters on it (C.R. 702.62a), even if it's an artifact, creature, enchantment, or land card. Indeed, an "artifact", "creature", "enchantment", or "land" means a permanent with that type on the battlefield (C.R. 109.2).
(In any case, no card currently has "suspend 0—[cost]"; indeed, part of suspend triggers when the last time counter is removed from the card with suspend while it's exiled, not when the card has no time counters on it while it's exiled [C.R. 702.62a].)
