Well if you are playing a slower mono red deck you definitely want inferno titan. It helps fight off tokens and aggro and is a strong finisher, combined with slagstorms you could definitely win against lots of decks. The matchup against control seems pretty poor, though (if they drop a wurmcoil your inferno doesn't do much), and Koth is one of the non-aggro red cards which really helps with that matchup.

It's quite playable, you probably wanna run him with ramp. Which in turn means you'd probably wanna fit him in a Wolf Run deck. The only real weakness there is having to deal with blue player's Flashfreezes on games two and three, though we have Wurmcoil Engine for that.
Then there's running him in a control build, like UR or RUG. RUG ramping into Inferno Titan is something you should look into.

Let's say I offered you a bet -- I'm going to flip a coin and if it's heads, I'll give you 1 dollar but if it's tails, you'll give me 2 dollars. This is obviously a terrible bet for you. Accepting it is a bad decision. You can't justify it afterwards by arguing that you won the flip, therefore you made the right decision.
If it wasn't a Titan, sure. If you mean tht you simply don't HAVE Koth, it's a more than adequate filler card. You're having an excessive gap in your mana, but it's a Titan.

The reason is pretty easy.. Inferno saw a ton of play when there were a lot of 3 color decks. The mana bases now push basically mono color or 2 color decks. 3 is doable but not reasonable to play double costs in the 3rd color.
So first mono red aggro doesn't want something so expensive. Big Red is rarely viable in a format that has prominent control decks and a better RG ramp deck. As long as there are decks that ramp that can win off their lands Big Red looks a lot weaker since they are fighting for the same place in the metagame. 2 Color enemy colors is awkward so the only decks that are 2 color that are going to play it are RG or RB. Surprisingly perhaps but there hasn't been a RB deck. Mainly it's because black really seems to like playing alone right now and any list that would go big enough to play 6 drops probably would go for Grave Titan.
RG is more interesting place. Wolf Run could run him but they seem to like their Wurmcoils and life gain. Any other deck in these colors first has to compete whether it's just a worse wolf run. Any dedicated ramp deck won't be so we're looking for the top end of a midrange deck. The tricky part is that white is a stronger second color in these strategies so you have GWr. Double R becomes harder here. Maybe in a pod deck that doesn't always have to cast it. It's still ambitious consistency wise. RGx seems like another option but white just has the cards right now so while I can see it finish in a RUG deck the RUG deck almost wants the Consecrated Sphinx but double red double blue is hard to do. Ultimately I believe it is mana and the lack of lands to use as mana sinks that led to Kibler sort of abandoning RUG.

Mythic red lost to much tempo when Chalice rotated...I played mythic red the entire time koth and chalice was together. Inferno titan just doesn't belong in Mono red without some kind of ramp, maby koth but chalice was where its at
Funny. I can't think of a better turn 3 creature to play in the current meta than inferno titan.
He works really well in heartless summoning decks, as the vast majority of his power isn't the fact that he's a 6/6 body, but that he's pumpable and pings everything everywhere, just playing him is usually enough to create a pretty significant board advantage.
He fits well in koth red, as the massive amount of red mana you can produce allows him to basically 1 shot your opponent the turn after you play him, and has the confusing ability to sort of 'get around' spellskite, though I'm not really sure I understand how that happens.
Seems bad to me in the meta. You're better off trying to get Elesh Norn into play for one more mana if you're worried about destroying little creatures.
I've played him in a Naya Pod deck.
He's outright won games for me (GW and Tempered Steel comes to mind), especially when I pod him from an Acidic Slime.
Putting him in a Mimic Vat is just plain hilarious too.

Big red is quite good actually. Probably not tier one but tumble magnet and slagstorm slow down aggro and multiple earlier threats overwhelm control. Check out my build in sig. 8 ramp cards and 14 real threats makes for a nasty deck. Turn 4 Koth ultimate is quite achieveable.
The problem with inferno (and frost for that matter) is that they do not produce permanents, which the other three do, and the nice thing about permanent production is that they are... well... permanent. 3 extra damage is nice, but if it said "put three 1/1 goblins with haste into play, you choose if they are attacking or not" then it would be a different story.
My friend plays a Big Red Geosurge deck with Inferno Titan in it. If we speculate just Inferno Titan, imo it doesn't do any good in this meta, but with the right deck it serves as a secondary threat.
