The Filters for that image went like this:
-Resize to 600 DPI (I started with the image in this resolution)
-Pixelate > Color Halftone, Max. Radius = 4 (the minimum)
-Resize to 100 DPI, and rotate ~5-10 degrees, to make it seem like it is more realistically "placed" on a scanner.
(now open a new document, at ~ 200W x 20H, and make a black/white gradient "bar" - this uses the fourth setting on the gradient tool. now go to Edit > Define Pattern, and set this as "scanlines". Return to the original image.)
-Create a new layer named "SCANLINES". Use the paint bucket tool, and set the fill to Pattern > scanlines. fill the entire layer.
-Go to the layer panel, and set SCANLINES to transparency = 18%, Blend Mode = Soft Light.
-Return to the original layer with the card, and make a selection in the shape of the card. Return to the SCANLINES layer.
-Go to Select > Inverse, and cut out the unnecessary part of the layer.
-Return to the Original layer, and go to Filter > Diffuse Glow. Set it to Graininess = 2, Glow Amount = 3, and Clear Amount = 8. (MAY DIFFER ON SOME COMPUTERS). Merge the layers.
-Now that the layers are merged, cut out any white around the card. Go to Layer > Layer Style > Drop Shadow, and set it to Blend Mode = Normal, Opacity = 100%, and Angle = 60.
Congrats, you have a crappy scanned fake!

First of all it has the stronghold symbol on it. Second of all you should include the entire card (border and all). And third of all you should probably have the card slightly on an angel and not perfectily verticle.

you want to change the symbol, and you want the "scan line effect" or else it wont look real. and it needs to be grainer, that blur is too smooth for a scan
@Hodoku: That's nice, but there are several issues. First of all, there's no way there would actually be a drop shadow on a scanned card. That's obviously shopped. Also, those scanlines don't really show up on most real low-quality card scans.

actually, crappy (old) scanners like mine do give a slight shadow, as well as those terrible scan lines. newer ones dont. and yes, photos would result in a shadow as well
(once i get back to my scanner) ill scan something as an example... i was working on that right next to a scanned image, and they did look similar.
edit: the REAL issue, is that the text is total crap. im actually trying to upload another fake that has the proper text (that was all just random characters).

@FPM: A photo of a card lying on a flat surface wouldn't have uniform drop shadow like what PS makes. The only shadows that would be made would be a result of curvature in the card, and thus the shadows themselves would be slightly curved.
@Hodoku: I stand corrected about the scan lines, I suppose. I've never seen a scan have a uniform shadow like that, though.
really? what happens, is that that backing on the scanner tends to "float" upward as the scanner is used more often (many times beacuse people scan thicker mediums, like books and suck, and is a bit off, resulting in a drop shadow similar to that one. like i said, once i get back to my real computer, i can scan one and show you
