Our clan is about to get a lot more active when we fuse with Mafia Theory. We do participate in things. We won the last Clan Mafia. MVP'd it even. (Generic did most of the work though)

Pretty much what ZDS said. Except, you can also wake people by gently touching them on head/shoulder while walking around the circle (you sit in circle formation). All targetting happens by pointing and GM gives results with thumb-up, thumb-down or crosses arms for "no results".
Also, days go bit differently, as there is no wagon analysis, vote count analysis or anything of the sort. You need to more rely on reactions and tone than anything. Also, trying to see some connections between people (like some people who just don't get on each others wagon) is a good way to go, as bussing happens less often (or at least, people usually don't start the day with turbo-bussing other mafia)...
The usual set up we have for 12-man game is
4 Mafia Goon
1 SK
1 Doc (can self-heal, no same target in row)
1 Cop
5 Town Vanilla
Sometimes we spice up things with Cupido (chooses lovers N0), Dayvig (both mafia dayvig and town dayvig), Twins (Mason without chat), Jailer, Silencer (makes someone voteless for next day, this is a really bastard role) and Grandmother (in smaller setups)...
Also, voting is bit different. When people want someone lynched, GM makes official count, where everyone who wants that player dead raises their hand. If there is 50% or more votes, the player about to get lynched get one last chance to save themselves and they are given a chance for a 1-2min speech (and if anyone else tries to disrupt this, GM modkills that player who disrupted). Then GM counts votes again and if it over 50%, then lynch happens. Otherwise, day continues as normal...
You can structure the day a bit by allowing players to make an official motion to propose a lynch. Arguments for are heard, then arguments against, then a vote is taken. Arguments could come from one player each (accuser and victim) or I guess by multiple people. Just means you don't have to keep track of votes all the time, and breaks the flow up a bit.
Well, yeah. It usually goes that someone proposes "how about we lynch X" and if people seem to agree, GM makes official vote count to see who supports and who doesn't.

In my experience, lurkers are more often mafia than not. And meta's usually reliable within 2-3 games.
Lurkers are mafia or newbies.
Also: Do no try to introduce mafia to the group of newbies with almost no experienced players. It's a bad idea.

I run last summer into this one guy, who thought it was a good idea to introduce mafia to new players in a convention, and had crafted a game for ~19 players, out of which almost all were newbies. It was open set up with following roles:
... the sad truth was, that that game was hosted like... 6-7 times and town lost every god damn time. Every. God. Damn. Time.
Mafia didn't have to do anything at all, just sit there and vote. No one wondered why certain few people always jumped on every potential wagon, and when I tried to call them out on it, everyone decided to lynch me for "trying to get someone lynched"... >_<
... but on the other hand, I once counterclaimed cop in a newbie game with open set-up (1 doc, 1 cop, 1 SK) and we ended up lynching the other player, who flipped cop. Wanna guess what happened? I made it to endgame and won the game as mafia, since next day, few of my mafia buddies just said "Well, I guess yesterday was just an accident, I don't think Bur meant real cop to be lynched" and no one even suspected me after that...
Certain players lurk because they have very little interest beyond spontaneous decision making. I can name at least two on this site but it's not a witch hunt.
That being said, the lurk is usually a chance for the scum to collect themselves after an attack and allow the heat to move away from them.
You need to also look at those who overcompensate for believing the the rule that lurkers are scum... Ain't that right ISO
