1 Hardened Scales and 1 Simic Initiate in play with 2 +1/+1 counters on it. Casting a Huatli’s Raptor and deciding to graft 1 from the Simic, does the Huatlis proliferate happen first or second or at the same time? The way I was thinking it would end up is the raptor would end up with +4/+4 and the Simic would end up with +3/+3.
Because both are triggers that trigger when a creature entered the battlefield and you control both sources you get to choose the order of the triggers. If you graft first and proliferate and choose the Initiate and the Raptor, the Raptor will end up with 4 +1/+1 counters due to Hardened Scales and the Initiate will have 3 +1/+1 counters. If you proliferate first then only the Initiate gets counters from proliferate leaving it with 3 and the raptor with only 2. Remember if you control the sources involved you choose the order when multiple abilities go on the stack at the same time.

122.6. Some spells and abilities refer to counters being put on an object. This refers to putting counters on that object while it’s on the battlefield and also to an object that’s given counters as it enters the battlefield.
