If I cast Eradicate, targeting one of my opponent's land creatures (eg. a Mountain), can I search that player's graveyard, hand, and library for all mountains and remove them from the game? (I assume that I can't remove all of their lands because only the lands IN PLAY are swamps, but the mountain cards in their graveyard, hand, and library have the same name written on them as the Mountain-Swamp creature that I targeted with Eradicate.
When Eradicate resolves, you search for all cards with the same name as the targeted creature, even if they're not creature cards.
Eradicate can target any "nonblack creature" even if it isn't normally one (C.R. 114.1a; see also this thread and C.R. 205.2b). And for Eradicate, you search for "cards" with a given name, not necessarily "nonblack creature cards" with that name.
Thus, for example, if the targeted creature happened to be named Mountain, you search the zones Eradicate says for all cards named Mountain (not necessarily all cards with the Mountain land type, such as Blood Crypt), even if those cards are not creature cards.
Note that neither Urborg nor Kormus Bell nor Blanket of Night changes the names of any objects, including lands on the battlefield. Thus, for example, a permanent named Mountain will still be named Mountain despite Urborg, Kormus Bell, and Blanket of Night.

This doesn't work, because Kormus Bell turns all swamps into black creatures (as per its Oracle text), and Eradicate cannot target black creatures.
I had not seen the Oracle text until just now. I am confused. The original text on the card, which you can read if you place your cursor over Kormus Bell) is:
"Treat all Swamps in play as 1/1 creatures... Swamps have no color; they are not considered black cards." How did this ever get changed to "All Swamps are 1/1 black creatures that are still lands" in the revised Oracle text??!!

It was changed in Revised Edition to omit the sentence "Swamps have no color; they are not considered black cards", and in Fourth Edition to say, in part, "All swamps become 1/1 black creatures", and the Oracle card reference generally maintains a card's functionality according to the most recent printing of the card, which in this case is the one from Fourth Edition. In any case, use the Oracle card reference to determine a card's wording (C.R. 108.1).

Yes, lands are colorless, so unless the effect adds a color (as Kormus bell does) they remain colorless when they become creatures.

So replace Kormus Bell/Urborg with Lifespark Spellbomb and you're set.
