Similarly, if I declared that I was equipping Leonin with the Greaves, and my opponent cast a Bolt at Leonin, could I instant-speed equip the Greaves to Leonin again, to give her shroud before Bolt resolves?

You are correct. The stack resolves in last-in-first-out order so the equip ability of Lightning Greaves will resolve before Lightning Bolt, equipping Kor Duelist and giving it shroud. After the Bolt has resolved, the equip ability of the Greaves targeting Leonin shikari resolves and the Greaves end up attached to it.

This works, but only because of the ability of Leonin Shikari. Otherwise, you can only activate Equip abilities when you could cast sorcery spells so you couldn't respond with it.

That won't work. For the non-First Strike combat damage step, the game checks whether the creature had First (or Double) Strike during the previous (i.e., First Strike) combat damage step, not during the current one.
510.5. If at least one attacking or blocking creature has first strike (see rule 702.7) or double strike (see rule 702.4) as the combat damage step begins, the only creatures that assign combat damage in that step are those with first strike or double strike. After that step, instead of proceeding to the end of combat step, the phase gets a second combat damage step. The only creatures that assign combat damage in that step are the remaining attackers and blockers that had neither first strike nor double strike as the first combat damage step began, as well as the remaining attackers and blockers that currently have double strike. After that step, the phase proceeds to the end of combat step.

I don't suppose you could find the exact ruling # for this could you? My friend says that you choose targets when casting a spell/using activated abilities/using triggered abilities, etc. and it would be too late to use Greaves to protect a creature with Leonin Shikari out.
Section 608 of the Comprehensive Rules deals with resolving spells and abilities. Within that section, you'll find:
608.2b If the spell or ability specifies targets, it checks whether the targets are still legal. A target that's no longer in the zone it was in when it was targeted is illegal. Other changes to the game state may cause a target to no longer be legal; for example, its characteristics may have changed or an effect may have changed the text of the spell. If the source of an ability has left the zone it was in, its last known information is used during this process. The spell or ability is countered if all its targets, for every instance of the word "target," are now illegal. If the spell or ability is not countered, it will resolve normally. However, if any of its targets are illegal, the part of the spell or ability's effect for which it is an illegal target can't perform any actions on that target or make that target perform any actions. The effect may still determine information about illegal targets, though, and other parts of the effect for which those targets are not illegal may still affect them.

12/1/2004: Leonin Shikari allows you to move your Equipment around during the combat phase or in response to a spell or ability. You're still subject to any other restrictions on activating equip abilities.
Combine those with Lightning Greaves giving its equipped creature shroud (and thus making it an illegal target for spells and abilities) and you get the effect described above, countering spells that had targeted the creature to which the Greavs are now equipped.

The game only cares whether the target of a spell or ability is legal at two times: when the target is first chosen and when the spell or ability resolves. It can become illegal and then legal again in between the two and the spell would still resolve.
For example, in 13055's first scenario, (the one where Kor Duelist is targetted by Lightning Bolt) if the player who cast Bolt lets the equip ability resolve, giving the Duelist shroud, and then casts Shatter to destroy the Lightning Greaves before the Bolt resolves, the Lightning Bolt will resolve normally even though its target was breifly illegal.
