Gideon's Phalanx is actually from Ravnica. The soldiers he's with are Boros.
Also, I'll move it, but I can't 100% agree to Gideon's Phalanx being in Ravnica due to how R&D and multiple articles keep mentioning that each walker gets 1 spell mastery spell for when on their home plane, 1 for when near they first walked on their first walked plane, and 1 more in their prime on their first walked plane. The buildings in the art as well do look somewhat Band in design, plus the number of trees. Then again, all three of Nissa's spell mastery cards look like their in Zendikar so maybe they fudged the cycles a bit for those two.
Those are, without a doubt, Boros soldiers though, so it has to be on Ravnica.
They are not soldiers, they are knight which makes me think of Alara before I think of Ravnica.

Hydrolash lacks obvious circles, true, but the mage is casting a double helix of water and a double helix is basically two strands 'circling' around a center point. It's probably too much of a stretch and not intended by the art team, but hey, I take everything I can get to keep my shaky theory up. :B
Looking at the flavor text, isn't obvious it's from Vryn?
The lack of rings is even explained, since the rebel hydromancer is "refusing" them
It's not just the rings. It's circles in general, as a visual theme across Vryn cards. Look at the "magical" effects on Thunderclap Wyvern and Separatist Voidmage.
Let me rephrase, when I hear knights I think Bant/Alara over Boros/Ravnica. It helps none of your examples are Boros.

Wow, I just wanted to say that you guys are amazing. I was really curious about this when Origins spoilers began, but I never thought it could really be made as you are doing it.

Before the glory of Yawgmoth, yes, even this makes sense.

Thank you, I appreciate the compliment. Though it is incomplete, it gives a great idea as to which cards are from which plane and how those planes are. Regatha is the only plane I think wasn't fleshed out enough in this set, only focusing on one area of the plane. But, I digress, if you have any card in particular your curious about feel free to ask.
Regarding Archangel of Tithes: In Magic Duels she attacks Liliana the moment she returns to her home plane after some necromantic studying on Innistrad.
UPDATE:
1. The evidence from the Magic: Duels game has made me lean more towards Archangel of Tithes being from Dominaria. I shall be moving the card there, if anyone opposes speak your peace.
2. In regards to fire = Regatha, I won't move them yet until I get another couple agreements. I'm just sad that the rest of Regatha wasn't shown, they have more than lava mountains. Like the forests with the Elves, and the main city where the Purifying Fire lives.
3. For the 3rd or 4th time, the art used for Divine Verdict on MTGSalvation is probably not the accurate artwork. Gatherer has Divine Verdict Magic: Origins as its usual old artwork, not the Theros artwork. Unless I can get some kind of confirmation that the physical Magic: Origins Divine Verdict is indeed the Theros art, I will not be moving it since my analysis is based off the non-Theros artwork.
EDIT: Finally decided to look up on Coolstuff as a 2nd reference which version is the art for Magic: Origins, it is indeed the old art work like I've been saying. I don't know who to talk to to fix the card being shown in the OP but the Theros artwork is not the artwork for the Magic: Origins Divine Verdict.
I poked around and I guess I was under the mistaken impression that "the Outlands" had been referred to in Theros, but there are definitely "outland" areas. He also has a clear starry sky in the background, which seems like a pretty obvious nod to Theros.
There is a reference to "giants and pit spawn" on the first printing of that art of Divine Verdict which would put it on...Rath? Pretty much plane-neutral.
A bunch of the reprints and general ones are going to be ambiguous, but here are the ones you still have as unsorted that I have feelings about.
Aegis Angel - not Zendikar due to halo position Alchemist's Vial - I think the actual colors in the art and the theme strongly hint at Kaladesh. Day's Undoing - Could be a reference to Lorwyn, Dolmen Gate in particular. Managorger Hydra - I think they are just leaves at the bottom, but they are reminiscent of the Theros starfield effect, and it kind of has a super-Heroic feel as well. Obviously tentative. Ramroller - Kaladesh is a strong possibility for this as well, based on theme and the attire of the people running (see Consul's Lieutentant) Watercourser - feels like the Lorwyn elementals
Otherwise I think you have the best candidates listed.
I'm a bit confused about the Dominaria section. Some of the cards aren't confirmed to be from there. Take Soulblade Djinn for example. We know Djinns (Djinni?) exist on other planes, for example Ravnica. I could also easily see Djinns to appear on Kaladesh. If Theros has dragons, I don't see why a loosely indian plane coudln't have east asian influences. Heck, it could even be from Vryn. *cough magical efffect looks like a circle cough*

With each decision I make I make sure to do at least 5-15 minutes worth of research scouring the cards of the past. Yes, there are Djinn on Ravnica, but they don't look like what Soulblade Djinn looks like. Out of the other planes only Dominaria has ever had Djinn. When it came to specific creature types like Djinn I didn't want to assume they were from a plane where it isn't confirmed they are from or not.
Fair enough, though I think absence of evidence does not imply evidence of absence. But hey, it's your list, so I'm the last person to tell you how to manage it. I was just confused, is all.

No problem, this list is meant to not just be my list, but a list of common opinion, I just got the thought train rolling. If there are any other cards your wondering about I'd love to explain my reasoning.
Also quick update on the OP. Under the Unknown Planes tab I've started to keep track of which cards are popularly thought to be on which plane with a + sign to make it easier to see what the general opinion tends to be.
So has explained in the article we have Zendikar (last forest), Vryn (first Island) but about others ?
Some are obvious:
Alara(Bant)= First plains
Theros= Third plains
Ravnica= Third Island
Innistrad= Fourth Swamp
But do you have ideas for others ? I'm particularly curious about the first and fourth islands and the first and second mountains ?
Thanks for your helps !
There's two planes shown on each basic land type, meaning that two cards per basic land type depict the same plane.
These are in order:
Bant (Plains 1 and 2)
Theros (Plains 3 and 4)
Vryn (Island 1 and 2)
Ravnica (Island 3 and 4)
Dominaria (Swamp 1 and 2)
Innistrad (Swamp 3 and 4)
Kaladesh (Mountain 1 and 2)
Regatha (Moutnain 3 and 4)
Zendikar (Forest 1 and 4)
Lorwyn (Forest 2 and 3)
Why they broke the pattern with the forests is anyone's guest.
Interestingly, one could say they broke the pattern with the plains.
The lands represent each walker’s birth world and then their spark world.
1&2 is the birth world, 3&4 the spark world. None of them (at that time) had returned to their birth world aside from Nissa, which is why she goes 1 -> 2&3 -> 4!
But this is clearly overthinking things as this doesn’t hold true for Gids =/ Still kinda cute.

"You can tell how dumb someone is by how they use Mary Sue"
You make Knight Watch cry.
And Knightly Valor. And Security Blockade. And Selesnya Charm. And Sunspire Gatekeepers. And Trostani's Summoner.
There has never been any doubt about this.

Outland Colossus is definitely Theros, the Outlands are a named location there and colossi are well established there.

We know that Liliana made her way back to Dominaria before making her deal with the demons because she already had her tiara (?) at that point.
Edit: Though I guess we don't know if she made other stops inbetween Innistrad and Dominaria, so that'd break the pattern anyway.
