To me, it seems like you can get either one single creature out of your library or your yard or both, but never two out of the yard or out of your library
It reads to me like you only get one creature in total. I'm pretty sure the main point of the 'and/or' clause is so you don't have to shuffle your library if you know you are just grabbing something from the yard. Lets you save time and/or not disturb known cards on the top or bottom that you'd rather leave there.
if you could take one form library and one from graveyard it would be worded similar to the red one that states "up to one sorcery and/or up to one instatnt
this would be worded "search for a creature in you library and up to une target creature card in your graveyard" this template is the same used for the planeswalker deck "tutor" that let you search in both place for one card, not one card from both places

The "and/or" clause is relevant because you may eshew searching the library (to not have to shuffle e. g. after a favorable scry) or search your library, but after not finding what you want there (maybe randomly exiled face down by another cards effect) you can still take the second best option from your graveyard.
Either way, you get only one creature unless the card says you search a creature card in "each" you library and/or graveyard. Without the "each" it's one card total.

No, that would be "you may search your graveyard for a creature and your library for a creature, put any of these creatures into play" . (I'm not sure about the exact templating for the final half).
As is written you may search 1 or 2 zones for a total of one creature.

It's not though, this isn't unfamiliar wording. "A creature card" is very specific.
