Geist of Saint Traft was from Innistrad, the first set, meaning he was already dead. Hey I'm just repeating what the wiki has to say about Withengar. As for the demons reforming usually they reform as different demons that may have a slight inclination as to their former plans or motivations.
Withengar ended the life of Traft while laughing, but his victory did not last long, since Innistrad's angels vengefully attacked him and destroyed his physical body.
Unfortunately, Withengar did not die, but reverted to pure black mana. Without being attached to the sword or any restrictive spell, it was only a matter of time for the demon to reincarnate again and feast upon devastation of the plane.

According to the wiki he was released from Elbrus when some Skirsdag tricked St. Traft into using it. Withengar killed Traft then a bunch of angels killed Withengar. However it also says that he reformed seemingly mostly intact (notably unlike other demons like Gris) during the events of DKA.

But honestly we don't know how Em is or isn't. In RoE she just got out and stretched, we didn't get any "character development." For all we know she fell into the sea and had a temple built around her while she naps. Pretty sure Ulamog made vampires while imprisoned; shouldn't be too hard for Em to make Illithids from behind the scenes. (Quandry, would they be merfolk clerics? Interesting thought.)
I told you the most cursed person in the MTG will be there. Liliana is on the second set. The big 5 will be there in all sets, or most of them.
After defeating the Titans; Nissa, Gideon, and Chandra were stated as staying on Zendikar to deal with the smaller Eldrazi. Unless something major happens, Jace should be recruiting, not moving them. Think of it like a superhero team-up. Everyone loves seeing Spider-Man with Hulk or Deadpool, but it should only be a few times a season.
You do know that there where eldritch things in magic long before the Eldrazi showed up, right? *All* the way in Legends there was a little creature called Cosmic Horror that was heavily inspired by Lovecraft, not to mention the Elder Spawn. And speaking of lovecraftian shoutouts there was the Serpent Warrior in Stronghold.
As I have mentioned many times, the original Innistrad block played much more with Lovecrafts themes and tropes than the Zendikar blocks ever did. And it did it better, much, much better. Heck, Ice Age had more actual lovecraftian horror than Zendikar (Whoo! Marit Lage). The Eldrazi are cool and all, but they are not lovecraftian in the slightest.
Ulamog was pretty clearly Galactus-based, hungry for everything and clearly wreaking havoc. Kozilek was a bit more like Godzilla, subtle at first, but then suddenly a force to be reckoned with, and wondered at (despite seeing the old movies, I still think of the 1998 movie as the best representation). Thus it leaves the door open for Emrakul, already canonically stated as the most powerful and oldest, to be the representation of Cthulhu. And especially on a plane known to already possess cultists and the like, my preference would be that we don't actually see Emrakul for at least two sets, because that would give the best sense of foreboding that should presuppose Lovecraftian mythos. Or, as you'd no doubt remind of again, Marit Lage.
The title including "Eldritch" definitely sounds like Emrakul will be around, "Moon" being there definitely ties in Tamiyo, and we already have flavor text bringing in Nahiri.
I'm very surprised at some of your assumptions. For instance, I've been talking about Jace seeking Liliana's assistance, not about Liliana seeking Jace. But if he asks for a favor, she could easily give conditions.
You talk about home planes and include Jace/Ravnica and Chandra/Regatha, seemingly ignoring the fact that their home planes are their first planeswalk. Gideon was from Theros, but was known as Kytheon, showing an emotional preference for Bant. Home is where you know where the lightswitches are, not where you were born. And Liliana spent time on Innistrad. We have no reason to believe she doesn't have a keep or hideaway.
You assume people know it was her and would recognize her. She killed some people, and there were some witnesses, but it wasn't like they took pictures. Description: white lady, black hair, has some tattoos that glow when she uses necromancy... oh, and she wears a purple dress.
It's like the witch trials of Innistrad, then?
Except not having a reason to return is, itself, a reason to not return for Liliana. She wouldn't want to waste time backtracking when she's got two more demons to kill, and the ghosts of a dead civilization breathing down her neck. She's not like some other walkers, who wander where they want, and show up on whatever plane, just because they feel like it. She's got very specific goals, and is extremely resolute in her pursuit of those goals. WotC would need to give her a very specific reason to return. And, as I stated, it needs to be something new, that she couldn't/wouldn't have taken care of last time.
Not sure if you missed the link from ThyLordQ, but in Unkindness of Ravens, she wanted to find a way to kill the other two demons without killing herself. That'd be a pretty selfish goal, so right in her wheelhouse. But she wanted Jace to help her figure out a way to do so. Last time Jace didn't want to find someone on Innistrad, so that's why she couldn't do it last time.
