-keeping the meta diverse (as long as a single deck doesn't threaten the popularity of the format)

The old cephalid breakfast strategy was to put every card of your deck in your graveyard then attack with an uber Sutured Ghoul (exiling 4 tarmogoyfs ) enchanted with a free Dragon Breath.
Ah, old days.
Except Dread Return is banned in Modern. Which means most of these decks will rely on Unburial Rites to do a much less powerful impersonation.
The closest I ever came to a Belcher style deck that milled itself in one sweep was a white deck using Endless Horizons to take out all your lands. It wasn't the best deck, but it was a cool trick to getting lands out and then setting you up for a big mill courtesy of the Spy/Informer approach.
Are there any lands in play or are we talking EVERY card is in the yard.

If the question is regarding ALL cards being in your graveyard, you have to have a little setup beforehand. Using mana sources that sacrifice themselves at least gives you something to work with. Past in Flames could be used after that. After casting it, all cards you have could be in your graveyard and you could start playing your combo with the leftover mana.

TEN TURNS REMAIN.
Assuming we're talking about a deck that intended to get to this point, Unburial Rites targeting Angel of Glory's Rise bringing back Azami, Lady of Scrolls and Laboratory Maniac is probably one of your better win conditions. But it does require 3W.
If you're talking about facing a mill deck, just play any deck that runs Emrakul, Ulamog, and/or Kozilek.

Like fatestitcher + 2(Gravecrawler) = 4 Vengevine for 2 mana.
I don't know if this would work in modern with the lack of lotus petal and dark ritual type cards but Balustrade Spy or Undercity Informer with zero lands in your library does the job in legacy.

From there if you have blasting station and Emrakul in your deck you can just 4 Horsemen the game
