I really like it as is but I kinda feel that it is pretty wordy. Not sure how to fix that. Up the mana cost and take out the non-land part? Also, the "or less" is pretty much just for confusion reduction, and could be cut if it is taking up too much space.
"Sacrifice" could be "exile". I know it is a minor nerf but due to the nature of that situation, it won't really change the gameplay much.

That wording swap technically works (I think), and amuses me greatly.
Hmmm, that Planeswalkers isn't too bad. His ult reads poorly, but it will likely just kill all opponents. His minus is very interesting. And his plus is a solid control ability. I hope we can see a Grixis superfriends deck in standard for this to fit into
Darksteel Mutation is a color pie bend. It should be a white/blue card. It gets by as being a bend because it is a Pacifismesque effect.
And flavor can be used to justify anything in any color. Mechanics are what determines a color. If you have a cool concept that has to be in a certain color, find a way in color to execute it.

I feel like a 4 mana 2/12 on it's own is kinda over powered. I'd bring the toughness down a smidge. Maybe 9? Still pushed. Other than that, I really like it. Flavorful, nostalgic, good card.

Can cards be transformed in other zones? It creates a lot of memory issues (Did they transform that or forget to flip it back after last game?) and weird rules issues (does it flip back if it gets shuffled in? What about discarded?)
Wouldn't a cleaner execution just be an alternate casting cost that causes the creature to transform as it enters the battlefield?
It still has the problem of shutting down 3/5ths of your colors and 0-3 of theirs. Getting to cast spells for free is cool, but for a ten mana enchantment that harms you that much, it's kinda boring. It's basically a much, much harder to cast and use Omniscience with a ton of drawbacks.

Svogthir is broken. Indestructible, gets bigger, and reanimates creatures from any graveyard for three mana? Compare it to Geth, Lord of the Vault. You could probably lose indestructible and be fine. It feels very similar to Simic, which would probably be an issue for the set, but not necessarily the card itself.
Mat' is great. Awesome flavor, strong card. Kinda wish she (they?) referenced tokens somehow.
Signing is a do nothing artifact that makes you jump through a bunch of hoops so that you can go get a card that screws you over usually. Note how most of the other cards that fetch specific cards also have at least some use other than getting the other card(s).
I think lifelink on vampires is more "I drain your vitality" than "I'm hungry bro". Haste and +N/+0 are usually what I associate with hungry hungry vampos.
I think it would be, at best, a heavy bend in black. Codec Shredder has a very similar effect as a precedent in colorless. Regrowth pushes this towards green, if any color. I can't think of any cards off the top of my head that get non-land cards out of your yard, but it feels right in colorless. Black getting instant, sorceries, and (to a lesser degree) enchantments back from the yard is strange.
